The payment of your bill is your legal obligation. You are also responsible for payment for services rendered to your spouse and/or children. The filing of insurance papers by Heritage Acupuncture staff is done as a courtesy and does not relieve you of this responsibility.
Insurance is not a guarantee of payment. If services provided are determined not to be a covered benefit by your insurance carrier, you agree to accept full financial responsibility. If your account is placed with an attorney for collection, you agree to pay a collection fee of one-third of the unpaid principal and interest (1.5% per month / 18% per annum), plus court costs, beginning 30 days after monies become due.
Returned Checks
Any payment made by check that does not clear your bank account will result in a $25.00 insufficient funds fee added to your account for each returned check.
Services to be Provided
Acupuncture serves individuals with a wide range of complaints including both acute and chronic healthcare issues. Treatments may include the insertion of sterile needles, bodywork, gua sha (rubbing of the skin with a smooth object), cupping (the application of glass cups with vacuum to the skin) and/or the application of heat and cold to the skin. You may refuse any of these techniques at any time.
Risks / Possible Side Effects
Treatment may result in certain side effects, including local bruising, slight bleeding, fainting, temporary pain and discomfort, and temporary aggravation of symptoms existing prior to treatment.
No Guarantees
I acknowledge that I have not received any guarantees or promises as to the results from the services provided.
Client Responsibilities
It is your responsibility to inform your acupuncturist of all aspects of your health, and as treatment progresses, to inform them of changes that occur. Please inform your acupuncturist if you are pregnant or suspect pregnancy. If you experience any pain, discomfort or possible adverse side effects, notify your acupuncturist immediately.
Medical Treatment
An acupuncturist is not a substitute for a medical doctor and will not suggest that you discontinue medical treatment. If you are currently under a physician's care, continue as long as your physician deems necessary. Consult your physician before altering any prescribed medications or treatments. In the event of an emergency, worsening condition, or new condition, consult a licensed physician.
Services to be Provided
Herbs are considered food supplements utilized to strengthen and support overall health and wellness. Herbs and nonmedical nutritional suggestions can serve as an excellent adjunct to a medical doctor's treatment, but are not a substitute for that treatment. An herbalist can neither diagnose nor treat disease.
Risks / Possible Side Effects
The historical record and modern research indicate that herbs most often used for healthcare have an exceptional safety record. However, adverse reactions can sometimes occur. Side effects occasionally reported include headaches, skin rashes, and digestive upsets — these generally resolve rapidly if the dosage is reduced or the herb is stopped. Should adverse reactions occur, discontinue the herbs and contact your herbalist as soon as possible.
Herb–Drug Interactions
Although clinically significant interactions between most herbs and prescription drugs are rare, some prescribed drugs have a narrow range of safe dosage, making any interaction more risky. Disclose all medications currently in use, including other herbs and supplements, to your herbalist and to your physician. Stop taking herbs at least 48 hours before surgical operations, and if prescribed anticoagulants, antiepileptic drugs, or digoxin until expert advice is received.
Toxicity
All herbs used are generally considered safe and non-toxic at recommended doses. Use herbs in accordance with dosage recommended by your herbalist. Disclose any past or current liver or kidney disease to your herbalist. Herbs should not be used during pregnancy or lactation without advice from a healthcare practitioner.
